Pre-EMS Activity Questionnaire

This form provides us with the personal information  we need to assess your level of fitness and should be completed prior to your first session.

Before undertaking an EMS sessions all clients must review these contraindications and confirm that they do not have a condition that would prevent them training.

All clients need to provide Fastrak with a Liability Waiver that confirms their fitness to undertake EMS
